[A] Budget: Looking for Proposals
[B] Scope of Service:
Vendor needs to provide level of care staff registry services to the government authority located in West Sacramento, CA.
A. Contractor agrees to provide licensed and certified registry level of care staffing to fill temporary vacancies, substitute STAR employees while on leave, provide temporary services when appropriate staffing levels cannot be maintained with civil service staff, or provide services when requested services are not available within state civil service.
1. Contractor must have sufficient registry staff to adequately provide comprehensive registry services to support operations and maintain appropriate staffing levels at STAR for all shifts, twenty-four (24) hours per day, seven (7) days per week, including weekends and holidays. If at any time STAR determines that the Contractor does not provide sufficient staffing for temporary relief services, as outlined above to provide appropriate covered services to STAR clients, STAR will require the Contractor to submit a corrective action plan within three (3) business days of notification
2. Contractor must have the capability to provide emergency registry service needs.
3. If an assigned staff person is terminated or otherwise leaves the registry, the Contractor will ensure that continuity of care is maintained by providing a replacement staff person.
4. Contractor agrees that, unless otherwise noted herein, all expenses associated with travel to and from STAR, lodging, and all training expenses for registry staff shall be borne by the Contractor and will not be reimbursed by the Fairview Developmental Center. Any and all services performed outside the scope of this Agreement will be the sole risk and expense of the Contractor.
B. Provided services will include basic and general level of care and behavioral training for individuals with developmental disabilities, following established standards and procedures. Duties will include, but not be limited to the following:
1. Assists clients in all activities of daily living. These duties may include lifting, positioning, and assisting with client mobility.
2. Administers medications and treatments. Records temperature, pulse, blood pressure, and provides basic first aid within the scope of duties authorized by the appropriate licensing authority.
3. Escorts clients on the facility and in the community, including to and from medical appointments, grocery shopping, transitional activities.
4. Observes and documents clients’ physical conditions and behavior and reports significant changes to appropriate staff.
5. Performs written and electronic documentation in electronic health record.
6. Maintains a safe, sanitary, and therapeutic environment, which promotes respect and dignity, and protects the privacy, rights, confidentiality, and physical and emotional well-being of clients.
7. Provides 1:1 supervision/direct observation of clients on special precautions.
8. Possible physical interaction with strong individuals who have issues with impulse control, aggression, property destruction and self-injurious behaviors.
C. Contractor will ensure as required by applicable statutes, regulations, and policies that all registry staff:
1. Possess at all times the required current license required to provide level of care nursing services in the State of California;
2. Have passed the CCL criminal background and fingerprinting;
3. Have passed a drug screening test, and
4. Maintain current CPR certification.
D. Contractor must provide STAR with proof of any required training, licenses, permits, certifications, background investigation, and fingerprint clearances, drug screening clearances, and any other required documents noted herein, for each of the registry staff providing services under this Agreement. Services will be performed only by registry staff meeting the requirements outlined in this Agreement. Services provided at STAR cannot start until the license verification process and approval, and background and drug screening clearances are completed by the STAR Program Director.
E. Contractor will provide STAR Program Director a contact number for requesting and coordinating of services. This must be a live person and not an automated voice mail system, during business hours of 7:00 am to 6:00 pm.
F. Contractor will provide STAR a designated after hours contact number and email to submit emergent request which the Contractor must confirm received during the next business hours of 7:00 am to 8:00 am.
G. Registry staff must wear clothes appropriate to the work environment which adhere to STAR policy. Safe and appropriate footwear is required. STAR shall not be responsible for responsible for damaged, lost or stolen items.
