[A] Budget: Looking for Proposals
[B] Scope of Service:
(1) Vendor needs to provide fitness for duty evaluation services to the government authority located in Sacramento, CA.
- The Contractor shall provide the appropriately licensed staff to perform Psychiatric and Psychoanalysis services that entitle the reviewer to practice such medicine, specifically Psychiatry and Psychoanalysis,
- Contractor shall provide the appropriately licensed staff to make a determination of the employee’s fitness for duty based on a review of all available medical reports, examination of the employee and any other pertinent consultations,
- Contractor shall provide the appropriately licensed/credentialed staff to provide Psychological assessments including assessment of cognitive decline, Physiological assessment of issues of physical decline, Substance Disorder assessment, Neurological assessment, and other assessments of miscellaneous types to determine if the employee would or would not be able to perform the essential functions of employment.
(2) All the questions must be submitted no later than November 5, 2020.

Performance of the work will be Onsite.
