Government Authority located in Meriden, Connecticut; USA based organization looking for expert vendor for COVID-19 contact tracer services.
[A] Budget: Looking for Proposal
[B] Scope of Service:
Vendor needs to provide COVID-19 contact tracer services will be responsible for assisting the local health department in case investigation and contact tracing of those who have been diagnosed with COVID-19 and who may have been in close contact with the infected person to the government authority located in Meriden, CT.
- COVID-19 Contact Tracer will provide health education and guidance to interrupt ongoing disease transmission, both onsite and telework will be required for the position, while maintaining requirements for client confidentiality, records, and data security.
- General Duties: Initiate prompt communication with people diagnosed with COVID- 19 and who may have been exposed to someone with COVID-19 (contacts) through the use of the state of Connecticut electronic ContaCT tracing system, Employ dynamic communication and interpersonal skills, cultural competency/sensitivity, tactful language, and empathetic interviewing skills to build rapport and maintain trust with patients of varied backgrounds, Verify the contact's identity during initial communications and prior to disclosing confidential information. Assure confidentiality and carry out efforts to locate and communicate with clients in a manner that preserves the confidentiality and privacy of all involved, Notify the contact of their exposure to COVID-19, following a script or guidance to provide COVID-19 health education. Conduct notification of exposure in a manner that maintains the confidentiality of the patient (or person who was diagnosed with COVID-19) and ensures that the identity of the patient is NOT disclosed, approved information and guidance on isolation and quarantine procedures and what to do if symptoms develop, Using ContaCT, ensure daily check-ins are completed to assess signs/symptoms. Follow up by phone with those who are not utilizing the text or email method, Collaborate with the Director of Health and Human Services, Associate Health Director, and Public Health Educator to efficiently complete contact notification and monitoring assignments, Participate in CT Department of Public Health virtual office hours on contact tracing and other required trainings and meetings and Maintain patient confidentiality and ensure that all data and information is collected in concordance with local data privacy and confidentiality standards.

[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location
