[A] Budget: Looking for Proposal
[B] Scope of Service:
Vendor needs to provide early intervention registered nurse services for up to 18 hours per week to oversee the Developmental Monitoring Program and provide Health Care Provider education of the Early Intervention and Developmental Monitoring Programs to the government authority located in Rochester, NY.
- County (County) has approximately 1600 Early Intervention children who reside in the County in various stages of case management. Approximately 200 of these children are enrolled in the Developmental Monitoring Program.
- Developmental Monitoring: this position will be responsible for contacting parents of children referred, describing the program and sending the parent periodic Ages and Stages Questionnaires and activities. They will need to document their communications and use excel to track monitoring efforts.

- Onshore (USA Organization Only);
[D] Work Performance:
Performance of the work will be onsite. Vendor needs to carry work in their office location.
