(1) Vendor needs to provide one or more options in-home services personal care, home
support, registered nurse, licensed practical nurse, home health aide, occupational therapist, physical therapist, speech therapist to the government authority located in West Chester, PA.
- Included whenever feasible and appropriate within a care planning process, and agency contracts for formal services are used to fill in gaps in care or to provide service when no other alternative exists.
- Other resources available to the consumer (such as Medicare, Medicaid, or third-party payments) shall be utilized to the maximum extent possible.
(2) All the questions must be submitted no later than January 28, 2020
