[A] Budget: Looking for Proposal
[B] Scope of Service:
(1) Vendor needs to provide nursing services for medically fragile students to coordinate, manage, supervise and medically oversee Licensed Practical Nurses for individual students who require individual medical care in school, based on their Individual Education Plan to the government authority located in Rochester, NY.
- manage the delivery of professional, and cost-effective continuous school health services on an as-needed basis, delivered by Licensed Practical Nurses (LPN), as outlined in the student’s medical care plan and mandated by the Individualized Education Plan
- The Agency must make every effort to hire the current private duty nurses who have worked with these students to date, in order to provide the best possible opportunity for continuity of care
- acquiring and maintaining all benefit costs, including Social Security, State and Federal Unemployment Insurance, Disability Insurance, Workers Compensation, and all other costs as required by state and federal law and providing proof of the same to the District
- The vendor shall be responsible for maintaining all payrolls, personnel and other records required by law
- provide staffing, coordination, management, supervision and medical oversight of the nursing staff provided to the students who require the services based on the Individual Education Plan
- Ensure continuity of care and existing relationships with school staff and families
- Maintain student confidentiality.
(2) A proposal conference meeting or call will be held on July 15, 2019.
(3) All questions must be submitted no later than July 12, 2019.
[C] Eligibility:
- Onshore (USA Only);
[D] Work Performance:
Performance of the work will be Onsite. Vendor needs to carry work in their office location.
