(1) Vendor needs to provide advanced primary care staff training service for improving the quality of care, reducing the total cost of care and improving the experience of care to focus on reducing unnecessary hospital and emergency department utilization, improving diabetes, hypertension and substance use outcomes and expanding the capabilities of the practice to deliver comprehensive primary care including integrated behavioral health to the government authority located in Baltimore, MD.
(2) A contract term will be for one year.

Performance of the work will be Offsite. Vendor needs to carry work in their office location.
