[B] Scope of Service:
(1) Vendor needs to provide public health nursing services to focus on a cohesive, forward-thinking team of professionals utilizing evidence based practices, working in tandem with the health department staff, treat all people with respect and dignity, be less critical and judgmental of others, develop a teamwork approach to daily work, promote continuous improvement, encourage continuing education and training and be accountable and accept responsibility, assessing and identifying populations at risk, developing and implementing health guidance, counseling and educational plans using established nursing processes, providing health plans to assure health promotion efforts that include primary clinical prevention and early intervention strategies, establishing procedures and processes to ensure competent implementation of prevention measures and treatment schedules to the government authority located in Mount Holly, NJ.
(2) All questions must be submitted no later than August 13, 2018.

[D] Work Performance:
Performance of the work will be Offsite. Vendor needs to carry work in their office location
